Introduction: :
The number of patients with complex wounds has been increasing during the hospitalization, presenting a significant challenge for frontline staff in managing these complex wounds. In order to overcome this challenge, it is crucial for frontline staff to receive proper wound management training. In our hospital, some of our nurses have been nominated to attended various levels of wound management courses. Based on the 3-tier service model for wound care in Kowloon Central Cluster (KCC), the second –tier wound nurse who has completed a recognized wound training course, is responsible for managing wounds that require advanced management within their own department. The third-tier wound nurse, who has completed Post-Registration Certificate Course in Stoma and Wound Care Nursing is responsible for managing of the highly complex or complicated wounds, and providing wound care consultation service within hospital.
One important component of the wound management training is the bedside coaching provided by the third-tier wound nurse. This proactive approach includes wound rounding on complex cases and focuses on enhancing wound management skills.
Objectives: :
1.Enhance the quality of care and further develop nursing professionals. 2.Enhance the competency and confidence of the second-tier wound nurse in managing complex wounds.
Methodology: :
1.The third-tier wound nurse conducts proactive wound rounding on complex and chronic wounds (e.g. stage 3 & 4 pressure injuries, unstageable pressure injuries, suspected deep tissue injury wounds). This rounding takes place every month with the second tier wound nurse. 2.Education is provided on different wound care products. 3.Upon admission, complex and chronic wounds are assigned to a designed second tier wound nurse. They monitor and share the wound condition with 3-tiers wound nurse during wound rounding.
Result & Outcome: :
Since August 2023, a pilot program for proactive wound rounding by the third-tier wound nurse has been implemented at medical wards. Five wound rounding sessions have been conducted until November 2023. During the 5-month pilot program, which involved 13 patients, 20 wounds cases have been classified as pressure injuries (stage 3,4, unstageable or deep tissue injuries wound) since admission. Feedback for the program has been gathered through an evaluation form that assesses the perception towards the proactive wound rounding. All ten second-tier wound nurses have provided their feedback. They unanimously agreed that the program enhance staff confidence in selecting difference wound care materials; improves staff knowledge on advanced wound dressing materials; and boosts staff competency and confidence in managing complex wounds. Importantly, the program is implemented without causing any stress or burden. Additional, they recognized that also agreed that the program’s framework facilitates high-quality care in wound management and is worth implementing.
