Role of Allied Health Professionals and Non-Medical Practitioners in Busy Macula Service

Aim: The talk aims to discuss the roles, allied health professions and Non-Medical practitioners play in a busy Macula Service at a tertiary referral centre in northeast of England (Sunderland Eye Infirmary) and in UK (NHS).


Methods: The presentation goes through the rapid progress in the field of Retina and development of new treatment regimens. This leading to increased pressure on safe delivery of the service in public sector. Work force pressures lead to some of these traditional medical roles being taken by Allied Health Professionals like Nurses, Optometrists and Orthoptists.


Training each profession brings its own challenges. Even though nurses seem like a natural profession to take up doctor’s roles, examining an eye can be difficult, as eye examination is a tough skill and requires using a specialist equipment. On the Other hand, Optometrists are naturally good at examining the eye but need training to perform any procedures including intra-vitreal injections. 


Currently similar roles are being taken up by various other science graduates including Ophthalmic Photographers and others and this lead to the development of ‘Ophthalmic Specialist Practitioner’ (OSP) role.


The talk also discusses the Ophthalmic Practitioner Training (OPT) and its structure established by Royal College of Ophthalmologists, London. Developing a multi-disciplinary team working environment can bring significant benefits to overall patient care.


Conclusion: The training, governance structure needed to develop the roles for AHP’s & Non-Medical Practitioners and effective implementation of these roles is explained.

Results of a Pilot Study Introducing Nurse-delivered Injection Service in Hong Kong

Exudative age-related macular degeneration, diabetic macular oedema and retinal vein occlusion causing macular oedema are all important and common causes of blindness. Anti-VEGF intravitreal injections are gold standard treatment for these diseases, and require multiple timely injections to achieve disease control and to restore vision. As the incidence of these diseases increase in our ageing population, our healthcare system has come under immense pressure to provide timely and sustainable intravitreal injection therapy for all our patients. 


To reduce the waiting time for our patients, we have introduced a new nurse-delivered injection service, which has been shown in other countries to safely and effectively increase intravitreal injection capacity. We developed a comprehensive training program for nurses, and designed a double-blind cross-over study that compared outcomes between nurse-delivered and doctor-delivered intravitreal injections. This presentation will present the results of the study, in particular the safety outcomes and patient satisfaction rates.

In 2022, the Ophthalmology Department of Kowloon Eastern Cluster (KEC) pioneered a training program to enable certified nurses to administer anti-VEGF intravitreal injections, as part of a pilot study to introduce nurse-delivered injection services in Hong Kong. 
 
Since its implementation, our trained nurses have successfully administered over 550 intravitreal injections. This presentation will share the department's experience in implementing and managing the nurse-delivered intravitreal injection service. Attendees can expect to gain insights into the specialized training program developed for nurses and the established injection protocol. The presentation will shed light on the benefits, challenges, and outcomes associated with nurse-delivered intravitreal injections, providing valuable lessons for healthcare professionals interested in enhancing patient care and optimizing workflow in ophthalmology departments.
