Introduction :
The potential risk of intravenous medication incidents was observed, Tseung Kwan O Hospital (TKOH) Nursing Services Division (NSD) included this topic in 2022/23 Risk Registry (Medication Administration) under nursing. Apart from medication safety training and sharing of medication incidents in various meetings, safety walk rounds were performed in one day area with chemotherapy service and one Surgical in-patient ward in 4Q 2022.
Objectives :
1. To identify the good practice and the risk about medication administration safety 2. Increase nursing staff awareness in medication administration safety
3. Create safety culture among nursing staff in TKOH
Methodology :
A cross-departmental team, including Hospital Chief Executive, General Manager (Nursing), colleagues from Nursing Services Division, Quality & Safety Office, Surgery department, and Ambulatory Services department, conducted a safety walk round focusing on intravenous drug administration. Observers used a tailored checklist to identify good practices and areas for improvement. Debriefing was performed right after the visit, and the reports were shared in the hospital and department Nursing Quality & Safety Subcommittee meetings.
Result & Outcome :
1. Satisfactory compliance with drug administration standards was observed.
2. Staff awareness of medication safety was heightened as the ward staff were greatly involved in the preparation of the walk round.
3. Good practices and suggestions for improvement were shared in various nurse meetings.
4. Improvement measures were followed up by related colleagues.
