Introduction :
Falling is unavoidable during children’s development, however accidental falls among children in hospital is preventable. Due to unfamiliar ward environment, pediatrics inpatients are more vulnerable to falls, which lead to many harmful consequences. The fall incident rate alarmingly increased from 0.22 in 2020 to 0.62 in 2021 in CMC Pediatric ward. A systemic review reveals that therapeutic play can promote children’s behavior and attitude during hospitalization (Godino-Ianez et al., 2020), therefore we worked with Hospital Play Specialists in designing educational materials. Besides, caregivers play a vital role in caring their children during hospitalization, it is important to involve them in preventing fall incidents.
Objectives :
- To promote paediatric inpatient safety - To enhance awareness of fall prevention among nursing and supporting staff, patients and caregivers
Methodology :
The awareness of staff, patients and caregivers were enhanced by conducting four activities: (1) a promotion month, (2) a series of educational materials (eye catching poster, educational cartoon and case sharing cards) were produced, (3) previous fall prevention materials (leaflet and Safety Ward Round Record Book) were reviewed, and (4) tailor-made souvenirs were produced. The safety practices are sustained in four areas: (1) Fall Prevention Safety Ambassador Team consisting of nurses and PCAs is set up to patrol in ward and deliver fall prevention information everyday, (2) Fall Education Record for each patient is developed to ensure patients and caregivers receive fall prevention education soon after admission, (3) small group training with case sharing to staff and CHAMPS referencing sample is developed to maximize the accuracy of early fall risk detection, and (4) regular audits on staff’s compliance and evaluation on patients’ and caregivers’ awareness and satisfaction.
Result & Outcome :
The project is launched in 2022 3Q, the overall fall incident rates is 0.30 in 2022 while it is lowered to 0.12 in 2023. Audits and surveys were conducted during Sept,2022 - Dec,2023. Audits on nurses’ fall risk assessment and spot-check on staff’s compliance on new fall preventive measures are done half yearly, the overall accuracy is 85% and compliance rate is 90% respectively. 540 patients and caregivers are interviewed on their awareness and satisfaction. 99.8% participants agreed that their awareness was enhanced and safety ambassadors had clear explanation. Over 60% of participants perceived that our newly designed educational cartoon and case sharing cards are the most effective educational material.
