Introduction :
Pressure injuries are essential burden to individuals, impacting their physical, mental and social wellbeing. Whereas pressure injury prevention and management are conventionally regarded as a nursing business, a multidisciplinary collaborative treatment team has been promoted as best practice. The occurrence of pressure injuries is considered an indicator of health care quality and safety. According to Nursing Quality and Safety (NQ&S) Annual Report, NSD, HAHO, the high incidence of hospital acquired pressure injuries in POH~0.895(the number of newly developed PI per 1,000 inpatient bed days occupied) as HA overall rate is 0.49 since 2018/19.
Objectives :
To explore the effectiveness of multidisciplinary collaborative treatment team in pressure injury prevention and management in 10/2022-2023 in POH.
Methodology :
Patients with complicated and non-healing pressure injury who hospitalized in medical department in POH from 2022-2023 are recruited to multidisciplinary collaborative treatment team: Geriatrician, Geriatric nurse, Wound Nurse, Occupational Therapist, Physiotherapist, Prosthetic and Orthotic Therapist, Dietitian, Speech Therapist, ward nurse.
Multidisciplinary wound round initiated on 10th Oct 2022, it would be held regularly in every alternative month. There are total 7 times of multidisciplinary wound round from 10/2022 to 2023, all professional parties would have delivered their professional input during wound round. Ward staff will have received that professional knowledge in caring patient afterwards.
Result & Outcome :
Total pressure injury rate in POH~0.20 which HA overall rate is 0.46 which showed in
Nursing Quality and Safety (NQ&S) Annual Report, NSD, HAHO in 2022-2023.
Quality improvement requires a multidisciplinary collaborative treatment team to succeed, and the need to build a cohesive and effective multidisciplinary team is critical for positive outcomes.
