Objectives :
To allow patients with chronic and stable musculoskeletal conditions to have options of earlier physiotherapy interventions with self management approach, empower patients for self-care and to shorten the waiting queue for Our Lady of Maryknoll Hospital SOPD physiotherapy service
Methodology :
After initial eligibility screening of patients, if patients disagreed to join, they will attend the OLMH SOPD PT service with their the original appointment date booked. If they agree, an additional new appointment for EK PT service will be booked after EK Dr I/C endorse and sign on the GCRS referral form
EK PT interventions will be provided and after discharge from EK PT, if further interventions from SOPD PT are needed, patient will cont with original appointment for OLMG SOPD PT service
If futther PT interventions not needed, the patients can choose to cancel appointemnet for OLMG SOPD PT service
Result & Outcome :
From 2 December 2022 to 30 November 2023, xx patients were recruited and x patients had completed the whole program.
