Introduction :
Lai King Building (LKB) is an extended care unit and provides infirmary, convalescent and rehabilitation service. Reform Rehabilitation Program (RRP) is a patient-centered program and aim to enhance a better patient experience through the smallest of change to promote home-liked culture, patient-centered care and Activities of Daily Living (ADL) engagement during hospitalization. The RRP includes 4 components such as: 1. Allow private food ordering and private pajama for patient 2. Greet patient with nickname program 3. Enhance patient’s ADL training on Sunday and Public Holiday (PH) program 4. Promote rehabilitation through smart device and video
Objectives :
1. Create home-like culture and patient-centered care. 2. Enhance and engage patient on rehabilitation training. 3. Enhance patient experience during hospitalization.
Methodology :
1. Allow private food ordering and private pajamas for patient 1.1 Standardize the workflow and distribute the pamphlet of private food ordering and private pajamas for promotion during patient admission. 2. Greet patient with nickname program 2.1 Ask and address patient for his / her preferred nickname and display in e-bedpanel. 2.2 Conduct satisfaction survey for patient and LKB staff 3. Enhance patient’s ADL training on Sunday & PH Program 3.1Enhance walking and transferring skills training to nurse and Patient Care Assistance (PCA) through lecture and assessment by physiotherapist and occupational therapist. 3.2 Nurse and PCA assist patient’s ADL training according to recruitment list on Sunday and PH 3.3 Conduct satisfaction survey of patient, nurse and PCA 4 Promote rehabilitation through smart device & video 4.1E-bedpanel display patient’s mobility and functional status - Enhance health care team in providing appropriate assistance for patient. 4.2 Robotic attendant recruited as a promotor - Promote “prevention of fall” through video playing to patient 4.3Video production on - 病人體驗 少改善 大心意 - Promote and share LKB RRP in Kowloon West Cluster (KWC) hospitals.
Result & Outcome :
1. Two program surveys are conducted to collect feedback in RRP. 1.1 Greeting Patient with Nickname Program Survey 40 patients and 50 LKB staff are recruited. All patients and LKB staff agree it can promote home-liked and patient-centered care and enhance patient engagement in rehabilitation. 1.2 Enhance Patients’ ADL Training on Sunday and PH Program Survey 14 patients, 23 nurses and 15 PCAs are recruited. All patients and nurses, and 94% PCAs show overall satisfaction with RRP and agree it enhances patient’s ADL training. Moreover, all patients, nurses and PCAs agree it provides continuity in rehabilitation. Also, it enhances communication between healthcare provider and patient/carer. 2. Video production on “病人體驗 少改善 大心意” had been launched through PMH facebook since November 2023 and the program was shared within KWC hospitals and positive feedback received. ‘RRP- Small changes to huge difference’ has showed the positive feedback from patient and staff. RRP is successful to make a huge difference patient experience through the smallest of change. In the future, innovative technology would be introduced to improve patient’s experience in rehabilitation training.
