Introduction :
The metacognitive focused occupational therapy (OT) program introduced by occupational therapists since 2020 aims to address the challenges of treatment compliance and outcomes in both in-patient and out-patient management for individuals with substance abuse issues. This specialized program is supported by the Beat Drugs Fund and focuses on early engagement and intervention to help clients cease substance abuse, improve their occupational functioning, and enhance their compliance with OT rehabilitation.

This program targets at building up rapport with clients, engaging clients immediately and instilling clients’ insight to current mal-adaptive behaviors in early in-patient phase. It serves to elicit clients’ motivation to cease substance abuse. Also, this could bridge and capture the golden time to enhance clients’ engagement so as to improve their compliance in OT sessions for further metacognitive therapy and occupational lifestyle redesign in out-patient phase.
Objectives :
This paper is to study clients’ treatment compliance, effectiveness and efficiency of OT service from August 2020 to July 2023.
Methodology :
A retrospective review was done on client compliance in attending OT intervention, pre- and post-assessment on self-efficacy of drug avoidance, frequency of drug use and occupational life functioning. Wilcoxon signed rank test was used to compare the self-efficacy in drug avoidance, occupational life functioning performance before and immediately after treatment sessions.
Result & Outcome :
There were 252 cases recruited to the program from August 2020 to July 2023. All client are able to bridge from in-patient phase to attend OT out-patient appointment after discharge. A total of 159 cases completed the program, with pre- and post-assessment and regular attendance in follow-up sessions. Eighty-seven percent of cases showed significant improvement in self-efficacy in drug avoidance (p<0.001). Eighty-five percent of cases reported reduced drug use frequency (p<0.001) and hundred percent of cases reported successful relapse prevention. Moreover, seventy-four percent of cases showed significant improvement in their occupational life functioning i.e. work, leisure, home-maker and social aspects (p<0.001).

Conclusion
In conclusion, the results indicated that early engagement and early OT intervention has improved client compliance and their occupational life functioning. This program could be beneficial to client and OT as it can enhance client engagement effectively and also improve the effectiveness and efficiency of OT intervention.
