Introduction :
Primary Nursing Care model has been adopted in PMH Paed&AM since 2008. Primary nurse will develop holistic individualized plan of care for each patient from admission till discharge. Therefore, primary nursing care approach can be adopted to offer appropriate Paediatric palliative care to patients with life-limiting and life-threatening illnesses. Engagement and empowerment of primary nurses on Paediatric palliative care are key to enhance Paediatric palliative care teamwork. In 2023, a nursing continuous quality improvement project was conducted to empower and engage primary nurses to provide appropriate palliative care to our Paediatric patients.
Objectives :
1. To empower & engage Paediatric primary nurses on Paediatric palliative care 2. To enhance nurses’ confidence in delivering Paediatric palliative care to patients and carers
Methodology :
A “Pre-test” versus “Post-test” study design was adopted. All nurses working in four Paediatric wards were recruited in the program. Training on Paediatric palliative care was conducted to the recruited staffs. Training included basic concept of holistic Paediatric palliative care, its importance with respect to HA vision, scope of service etc. and two case sharing on symptom control, psychosocial & spiritual support, family support, bereavement/end of life care and team approach etc. An eight-questions pre- and post-staff training questionnaire (six-point Likert scale) with focus on concepts and knowledge of Paediatric palliative care, staff confidence & staff engagement was designed. Post-staff training questionnaire was conducted right after the training. Besides, a resource file with community referrals & information on palliative care was developed for easy retrieval by Paediatric primary nurses.
Result & Outcome :
From June to July 2023, total 113 nurses had completed the training (APN 20%, RN 80%). The mean score of pre- and post-training staff questionnaire had increased from 3.66 to 5.33. The mean score on staff’s knowledge & confidence had been increased from 3.41 & 2.9 to 5.30 & 5.11 respectively. Regarding staff’s engagement, mean score had increased from 4.26 to 5.48. Conclusion: Enhancement of Paediatric palliative care teamwork can be achieved by an empowering and engaging training program through primary nursing care approach.
