Introduction :
Hemodialysis (HD) centre was planned to move in to the new building of Kwong Wah Hospital on 21 May 2023. It was anticipated that HD patients and carers would experience with anxiety due to unfamiliar with the new environment, new HD location and routing. In order to enhance their familiarization with the new patient journey in the new hospital, a photo-guided instruction leaflet providing image routing for HD patients and carers was created.
Objectives :
This photo-guided instruction aimed to minimize the anticipated anxiety of patients and carers, enhancing their familiarization with the new patient journey. Consequently, they could attend the HD sessions as scheduled without delay.
Methodology :
Created a photo-guided instruction leaflet on new patient journey categorized by pre-HD and post-HD attentions with image routing attached. The instruction leaflets were distributed to all HD patients or carers accompanied with staff explanation 2 weeks before move in to the new building of the hospital.
Result & Outcome :
A total of 98 HD patients or carers received the photo-guided instruction leaflets. All of them claimed lessened anxiety and enhanced confidence to undergo HD in the new building. They could attend the first session of scheduled HD on time without hesitations and demonstrated adherence to the new patient journey smoothly in the new hospital. Moreover, renal nurses could guide them to complete the HD sessions successfully.
