Introduction :
During the COVID-19 pandemic, guidelines on infection control and prevention for COVID-19 has been updated by World Health Organization (WHO) and American Heart Association (AHA). Therefore, our department has also followed the resuscitation guidelines so as to practice evidence-based medicine and reduce the infection risk to our staff.
As received feedbacks from our staff at different ranks, the items in the General Outpatient Clinic (GOPC) emergency trolleys and out-reached bags are redundant and are rarely required in actual GOPC emergency or outreach situations. Nurses are checking all items weekly even though some items have been never used.
Objectives :
Review and update resuscitation guidelines, equipment in emergency trolley and out-reached bag, and CPR drill to improve quality of CPR and reduce infection risk during resuscitation
Methodology :
Updated guidelines and protocol of WHO Major trauma backpack 2021 and AHA Basic and Advanced Life Support 2020 were adopted.
KWC FM&PHC Patient Safety and Resuscitation Subcommittee has conducted the audit on resuscitation drill in 15 GOPCs to ensure the quality of resuscitation process.
The contents and format of resuscitation drill reports for all KWC GOPCs has been standardized with more items such as “Time to arrive resuscitation room”, “Time of shock”, “Time for adrenaline injection” etc. were added in the Checklist of resuscitation drill. We all encourage attachment of photos and diagrams to highlight individual clinic’s local CPR environment, team work and different rank cooperation were highlighted.
Each item in the emergency trolley and out-reached bag has been reorganizeded in different drawer to facilitate access during resuscitation. The redundant items such as excessive types of angiocatheter, number and types of intravenous fluid, number of types of intubation equipment were simplified. The defibrillators are gradually changed from model “Philips Heartstart FR3“ to “Cardiac Science PowerHeart G5” in all KWC FM&PHC GOPCs, with related instruction of usage and training provided to nursing and medical staff.
Result & Outcome :
All KWC GOPCs have performed the resuscitation drills with reference to the most updated resuscitation protocol, the process of the resuscitation has been reinforced, items in the emergency trolley and out-reached bag has been simplified, recording form is clarified, in order to deliver an effective resuscitation at the lowest risk of infection.
