Introduction :
In Hong Kong, the prevalence rate of people aged 65 or above with dementia is 7.2 % in 2018 (Wu et al., 2018)
According to the snapshot information of a Medical & Geriatric ward in Shatin Hospital (SH) in March, over 90% admitted patients are aged 65 or above and 28.6 % of them are diagnosed with dementia.
Some of them might feel distressed and developing behavioral and psychological symptoms of dementia (BPSD) in an unfamiliar environment after admission (Digby, Lee & Williams, 2017).
SH is implementing restraint free program in order to reduce the negative impact caused by restraint. Therefore, series of effective dementia patient care program is vital to deal with the different needs of dementia patient.
Objectives :
1. To facilitate the dementia-friendly and physical reduction culture in SH
2. To promote person-centered care to dementia patients, minimize the BPSD severity
3. To empower staff in taking care dementia patients
Methodology :
The core of the dementia care program is to issue the “Dementia passport”. We can invite patent’s carer to fill in the “Dementia passport”, the information inside about the patient’s background, preferences, interests and routine can help to facilitate the person-centered care.

Also, we sought the fund from HRC and management in preparing different kind of dementia toys for example baby dolls, coloring and practicing books, folding paper etc. We also added technological elements in the dementia care, we use the robot and projector plus to provide entertainment such as playing music and movie as diversional therapy. Thus, nurses can provide appropriate toys or activities to patients based on the “Dementia passport”

Meanwhile, nurse might not available to provide diversional therapy all the time since the busy workload, so we cooperated with the volunteer team to have a Dementia Buddy Scheme that they could play toys with patients when they come to visit.

Furthermore, we also held the weekend group activities for the dementia patients and their family like guess riddles. So, patients could enjoy the time together with their family and also help to relieve their distress. At the same time, patient’s relative can learn the caring tips of using diversional therapy.
Result & Outcome :
Pre-post assessment of BPSD severity was done by using the Neuropsychiatric Inventory Questionnaire, it had significant improvement.
Over 80% of patient’s relative and 90% ward’s staff agreed the dementia care program can relieve the agitation of dementia patient in the survey. 95 % ward’s staff agreed that it can help to relieve their stress on take care dementia patients in wards.
