Introduction :
There are 3 Outlying Island (OLI) General Out-patient Clinics (GOPCs) in Hong Kong East Cluster (HKEC), including Peng Chau (PC), North Lamma (NL) and Sok Kwu Wan (SOK) GOPCs which provide First Aid Post (FAP) service to local residents in addition to general out-patient clinic service. Since there is no hospital setting in island, PC and NL GOPCs also operate 24 hours FAP service. The average attendances of FAP are over 700 cases in 3 OLI clinics every year. It is a great challenge for nursing staff to cope with emergency situations after office hours without medical staff on duty. Nurses are expected to have high competence to manage emergency cases e.g. obstetric, trauma, resuscitation cases and air medical transport.
Objectives :
1. To equip and enhance competence of nursing staff in emergency care management
2. To improve nursing assessment and documentation for FAP cases
3. To evaluate the effectiveness of the enhancement program
4. To provide safe and high quality emergency service for local residents in OLI
Methodology :
A focus group was set up to review the FAP cases in the past years and explore the areas to enhance staff’s knowledge and practical skills. Tailor-made enhancement program was conducted from 2018-2023 to focus on the following aspects:
1. Lectures and simulation workshop on emergency care in OLI GOPC
2. Basic obstetric emergency care workshop
3. Advanced life support crash workshop
4. Air medical transport workshop
5. Regular CPR workshop and drill
6. Standardization of nursing documentation for FAP cases
Result & Outcome :
Over 40 nursing staff attended the training and participated in the surveys to evaluate the effectiveness of the program and staff competence level in emergency care. The results showed significant improvement and strengthening in knowledge, practical skills and confidence in managing emergency care in out-patient setting.

All nursing staff members were satisfied about the simulation training program, enjoyed the learning process and enhanced their competence in emergency care management.

Regular review and enhancement training programs are essential for OLI nursing staff not only to enhance their knowledge and practical skills but also to ensure efficient, safe and high quality emergency service for local residents.
