Introduction :
Nurses in Delivery Suite (DS) are responsible for providing peri-anaesthetic care to pregnant women undergoing obstetric operations such as Caesarean section, Manual Removal of Placenta and Uterine Exploration…etc. Historically, the training of peri-anaesthetic care is only by skills transfer from senior nurses. In order to enhance the competence of nurses in peri-anaesthetic nursing and to ensure a high standard of specialized care for clients undergoing anaesthesia or sedation, a structural training program was developed and implemented in July 2022.
Objectives :
(1) Standardize the post-anaesthetic nursing care for obstetric patients in DS
(2) Enhance nurses’ knowledge, skills and ability on anesthesia and post-anaesthetic care
(3) Minimize post-operative adverse events
(4) Facilitate the transition from paper documentation to electronic documentation
Methodology :
3 midwives of DS were identified as the trainers of peri-anaesthetic nursing. They had been arranged to have 1-day clinical secondment in main operation theatre for post-anaesthetic care training, then followed by completion of PRCC fundamental module in anaesthetic and recovery nursing. Training materials, including department guideline of post-anaesthetic care in obstetric patients, quick reference on post-anesthetic care in recovery room and photo guides on the environment of recovery room and emergency call system, and training plan for DS nurses were developed. All nurses in DS have to complete the assessment on the post-anaesthetic care. For new comers, the training on post-anaesthetic care was incorporated in the orientation program and related training would be started from the 4th week of entry. New comers have to provide post-anaesthetic nursing care on at least 3 cases under supervision and complete the assessment by the 7th week of entry. While on documentation, hands-on training on EZ Recovery were arranged. Effectiveness of the training would be evaluated by the compliance rate of assessment, and random checking on documentation in EZ Recovery by supervisors and trainers. In addition, survey was conducted in regard to nurses’ perception on implementation of the training program.
Result & Outcome :
As at December 2023, 54 nurses were trained and passed the assessment on post-anaesthetic care. Satisfaction survey was conducted in February 2024 and all the trained nurses had participated in the survey. They all agreed that the training program is systematic and providing them a clear guidance on post-anaesthetic nursing management from admission, monitoring to discharge phases. The training program gains positive feedback from staff and is an effective tool to enhance nurses' competence on anesthesia and post-anaesthetic care. In future, the program will continue and explore further cross-department experience sharing and interactive training in peri-anaesthetic nursing.
