Introduction :
Introduction: Accurate and up-to-date nursing forms are essential in capturing clinical information to evaluate the patient’s treatment effectiveness and to facilitate the communication among healthcare professionals. However, it is identified that there are 27 out of 32 forms, related to nursing, being used for more than 6 years without reviewing. It may induce discrepancies with guidelines and polices, which may pose a potential risk to substantial nursing care. With the opportunity of e-Vitals implementation in HHH, there is a need for an effective nursing form review process to ensure up-to-date documentation, eliminate the redundancy of the forms, promote communication and enhance patient safety. Additionally, the streamlining and simplifying nursing forms can allow staff for a great focus on clinical activities, thereby improving nursing operation efficiency and ensuring safe practices.

Objectives: (1) To update the content of nursing forms; (2) To simplify the forms by integrating the similar forms or eliminating duplicated;
Methodology :
Methodology: In June 2023, a nursing form review workgroup was established. Workgroup members including APNs, WMs, DOMs to review all the nursing related forms and updated the contents based on the latest guidelines and evidence-based information as necessary. A preliminary version of the updated forms would then be circulated to doctors, DOMs, WMs and NCs for feedback. Once the final version of the updated forms was endorsed, the briefing sessions were conducted for frontline staff to explain the rationale behind the changes and provide guidance on the usage of the forms.
Result & Outcome :
Result: 32 nursing forms were reviewed, including 11 updated forms, 16 obsolete forms and 5 forms that were reviewed but required no changes. Additionally, 7 new forms were created to meet the operational need. Total 23 forms were addressed as at Jan 2024. The feedback from staff during the briefing sessions was positive and found it to be highly beneficial for daily work.
Outcome: The nursing form review exercise effectively enhanced the awareness of proper documentation practices and increased nursing efficiency by reducing the time required to complete different forms. The updated and streamlined integrated forms also provided the clear clinical information among healthcare professionals in which ultimately improving the overall quality of patient care. Regular reviews of nursing forms also serve as positive step towards hospital accreditation preparation in the future. Therefore, the nursing forms review exercise should be encouraged as a part of ongoing quality improvement efforts.
