Introduction :
Sepsis, a systemic dysregulated host response to infection, leads to multi-organ dysfunction, failure and even death. Given the high prevalence and mortality rate of urosepsis, early recognition and handling of patients with urosepsis play a vital role. In Pok Oi Hospital Operation theatre, 15 cases of patient conditional change cases were recorded in 2022, and cases developed urosepsis and required unplanned post-op ICU occupied 10. The proportion of those cases was 67%. It can be seen there was a high urosepsis incidence rate in POH OT. It is found that there were no checklists, guidelines or training about urosepsis. Moreover, nursing staff with less than 3 years OT experience accounted for 32.9%, and 11 new nursing staff joined us in 2022. The ratio of junior nursing staff was high.
Objectives :
(1)To enhance the competency of nursing staff in handling uroseptic cases in OT in order to improve patient safety.
(2)To improve nursing staff’s knowledge of urosepsis.
(3)To create a checklist of urosepsis as a quick guide for nurses.
Methodology :
According to The Focus, Analyze, Develop and Execute model, we focus on the importance of training for nurses about urosepsis management. Second, we analyze the level of training needs, competence level and their views by self-reported questionnaire. Finally, we design a workshop about urosepsis and pre/post test to evaluate the nurses’ knowledge level and competence level.
Result & Outcome :
In the post-test, the correct rate of each question was more than the pre-test. The average score of the post-test increased 4 marks out of 10 questions when compared with the pre-test. Staff showed significant improvement in answering questions related to both knowledge aspect and management aspect of urosepsis, which was 44.9% and 33% respectively. All participants showed agreement in knowledge improvement enhanced their competency.
The workshops were effective in that it could help participants to increase knowledge, sense of early detection and competency in urosepsis cases in order to facilitate the fast-paced working environment in OT, provide safe and effective patient care. Also, a checklist, which is a sustainable practice, was established and placed in PACU to help staff to get quick refreshment and guidance.
