Introduction :
For accomplishing digital transformation in North Lantau Hospital (NLTH), clinical handover in Extended Care Ward (ECW) was implemented by using both electronic documentation (e-Doc) and cluster-based “Patient Clinical Hand Over System” (PCHOS) since December 2022, together with e-Vital & e-Bed panel systems, digital ward place, and clinical dashboard to easy access the patient information which had enhanced the efficiency of clinical works in extended care ward. Staff satisfaction survey had been conducted to evaluate their acceptance and efficiency by adopting digital smart care projects in ECW.
Objectives :
1. To better understanding staff acceptance and level of satisfaction after implementation of digital transformation projects in ECW, especially for nursing clinical handover.

2. To better prepare the integration of different digital smart care projects in ECW for the next phase.
Methodology :
A series of in-house trainings, individual peer coaching and support, structural documentation templates were provided to the frontline nursing staff. Interval evaluation in documentation and efficiency of handover duties audit were performed after implementation of projects. Evaluation had been conducted from three aspects:

1) A cross-sectional online survey was conducted to all ECW nursing staff. A questionnaire was developed. There are 9 questions with rating scales and 3 open-ended questions.

2) Collaboration with Hospital Information and Record Department (HIRD), the quantity of Patient’s paper record were estimated before and after implementation of e-documentation for the comparison in monthly basis.

3) Time for searching patents’ record folder for information was compared before and after implementation of e-documentation to estimate the time consumption.

Total 37 questionnaires were collected from ECW nursing staff in Jan 2024. 94.5% nursing staff agreed that electronic documentation could save time and more efficient to perform nursing care. It also saved 10-15 seconds for searching patients’ record folder which less than before. 97.3% nursing staff agreed that electronic documentation for handover across different shifts were more consistent and accuracy. 86.4% nursing staff agreed that it improved communication with different disciplines and could facilitate real-time information for expedite patient flow. 97.3% nursing staff found that it could promote paperless health records. Moreover, the patient’s record folder lost 50% weighted less than before the implementation of e-documentation. Overall: 81.8% nursing staff agreed that electronic documentation was user friendly.

Conclusion:

To conclude, most of nursing staff were showed satisfaction and acceptance of adopting digital smart care projects in ECW. For the way forward, staff had suggested to enhance large font in e-Doc, and enhancement in the acknowledge function of treatment editing and adding. Although with the advantage of adequate “built-in” hardware for smart care, more iPads or accessible CMS and display dashboard were recommended. Different enhancement and integration of smart care projects would be foreseen, more in-house training workshops & review of clinical workflow should be organized to enhance the overall patient journey and direct quality care to every patient.
