KCC FM&GOPC Train-The-Trainer(TTT) workshop to enhance the standard & quality of family medicine training

Introduction :
Hospital Authority is the major location providing family medicine specialist training in Hong Kong. Family medicine trainers are tasked with fostering an environment that enables trainees to acquire new knowledge, skills, and competencies. With the increased family medicine trainees allocated to our Department, ensure the standard of family medicine trainers would be essential to ensure the quality of family medicine training. TTT workshop has been held to enhance the standard and quality of our trainers with introduction of various training modalities and updated knowledge of family specialist examinations.
Objectives :
1. To maintain family medicine training trainers’ standard

2. To update family medicine trainers ‘understanding of family medicine specialist examinations

3. To enhance Consultation Skills Assessment(CSA) training by assisting participants to acquire the ability to:

• assess and calibrate the consultation performance of family medicine trainee in a systematic manner

• accurately identify and prioritise trainees’ consultation strength and area for improvement

• select appropriate strategies to assist trainees to improve their consultation competency

• deliver feedback structurally and effectively

4. To learn and teach with other training formats: Random Case Analysis(RCA) and Problem Case Analysis(PCA)
Methodology :
The TTT workshop consists of 4 sessions of intensive training workshop and family medicine seminar attachments. Besides, it contains optional exit seminars attachment and FHKAM(FM) exit exam Practice Assessment(PA) mock exam attachment.



The contents of the workshop are as below.

• Introduction to Leicester Assessment Packages(LAP)

• Standardisation of CSA with standard consultation videos and trainees’ consultation videos

• Introduction to effective feedback

• Introduction to RCA

• Introduction to PCA

• Demonstration and practice of PCA with standard training videos and trainees

• Updates of FHKCFP exam formats

• Updates of FHKAM(FM) exam formats



The workshop was held by family medicine trainers with credentials of TTT workshop level 2 and level 3 experiences. The workshop will include a presentation of standard materials. And participants will view the standard training videos and trainee’s consultation videos together to align the standard of training. Besides, participants will practice feedback of CSA and PCA with trainees together. Comments on the feedback and PCA will be given to the trainer.



Pre-course and post-course evaluation were done to review participants expectations and the effectiveness of the TTT workshop. A scoring scale from 1 to 6 and True/False questions had been used to assess the effectiveness of TTT.
Result & Outcome :
6 trainers with various training experiences were invited to participate in the TTT workshop held from April to November 2023. Their training experiences varied from one year to more than 10 years.



After the TTT workshop, the participants have increased in confidence to use LAP to review consultations with their trainees (Pre vs Post means: 3.17 vs 4.67). They also have increased level of confidence to feedback their trainees effectively (Pre vs Post means: 3.67 vs 4.67) and effectiveness to feedback current trainees (Pre vs Post means: 3.67 vs 4.5).



Only 1 trainer knew RCA and PCA before TTT. And all participants did not know RCA would be the most appropriate method to review a series of trainees’ referral indications before TTT. TTT introduced new training formats to the participants and allow them to use most appropriate training method to train their trainees.



After TTT, all participants increased their familiarity to family medicine specialist examinations (Pre vs Post means: 4.17 vs 4.5). Before TTT, only half of the participants knew the new format of FHKCFP exam Applied Knowledge Test(AKT) segment and FHKAM(FM) exam Pre-Exit Review of Medical records(PERM) format. And only half of the participants knew the updated requirement of FHKAM(FM) exam CSA segments. About half to two-third knew the updated requirements of FHKAM(FM) exam PA segments. After TTT, all participants had updated knowledge about the FHKAM(FM) exam.



All participants agreed TTT could fulfil their expectations. All participants agreed TTT could fulfil their expectations. The expectations of the participants included application of LAP, enhance skills and update knowledge on training, improve communication skills with trainees and feedback skills etc.



KCC FM&GOPC TTT workshop could enhance the standard and quality of family medicine trainers with introduction of various training modalities and updated knowledge of family specialist examinations.
